Directions for Enrollment

Bring the completed forms and information listed below to the counseling office to enroll your student at Dunbar.

Completed Enrollment form

Completed Household Registration Form (English version or Spanish Verson)

Completed Language Survey form (English version or Spanish Version)

Copy of immunization records

Copy of birth certificate

Copy of Social Security card

Proof of residence, which may be any of the following:

Current utility bill (preferable)

Copy of lease for an apartment

Copy of purchase contract for a house

Copy of transcript from previous school if available
